This complaint is on behalf of my child, [insert name]. My child is enrolled at Chicago Public Schools at [insert school name].
According to my child's IEP, s/he requires [insert # of minutes]. The service minutes are in [insert service area such as Special Education, social work, OT, Speech, PT listed on the IEP]. My child receives services in order to receive a FAPE.
Having failed to deliver these services since [insert dates], CPS has not met it's FAPE obligation to my child. CPS has not met its FAPE obligation for [insert # of weeks].
I request that Chicago Public Schools provide compensatory services by making up all missed service minutes.
